Congratulations on your decision to apply for Co-op. Please click on the appropriate button below.

Students who are currently working as co-ops and wish to renew for the Spring (or request an extension to work Dec 21 – Jan 9 only), can submit their Work Term Evaluations and on-line applications to the CMC after Nov 9.

Students who plan to begin a coop or internship this Spring can make an appointment to meet with their CMC advisor after Nov 23, to submit their forms for approval and processing.

Cooperative Education Program First-Time Registration Instructions

Please complete Steps 1-3 below to register for the Cooperative Education Program. All paperwork must be signed by the student, the faculty advisor and the work-site supervisor before it will be accepted by the Career Management Center. NO copies or faxes will be accepted. A job offer letter that includes your start date, your title and your salary, and a detailed job description must also be submitted with this paperwork. International students, please make sure that you complete the Faculty Advisor Form with the added CPT Recommendation Form in Step 1. If you are changing employers or if you have completed one degree program at IIT, and are beginning a new degree program, but continuing your co-op, please fill out the paperwork in steps 1-3, and contact your CMC Advisor to find out if any additional paperwork is required.

Students must attend an Introduction to Cooperative Education & Internship workshop before applying for a co-op or internship.

First Time Co-op Registration Forms:

Main Campus Students

Step 1 Faculty Advisor/CPT Recommendation Form - International students only -OR- Faculty Advisor Form - U.S. Citizens & Permanent Residents only	The student's academic advisor must approve the student's academic standing, offer letter and job description before signing this document.
Step 1.B Academic Standing Exemption form	Download and submit to the CMC if you are a graduate student and your GPA is below 3.0.
Step 2 Student Co-op Forms - Required for allstudents	NOTE: The Student Co-op Plan must be signed by the student, faculty advisor and work supervisor. The form will only be accepted if allsemesters of co-op and classes are filled in. (Please see Sample Student Co-op Planfor instructions.)
Step 3 Employer Co-op Forms - Required for allstudents	It is the student's responsibility to provide these forms to the employer to complete.

Cooperative Education Program Renewal Instructions

Steps for Co-op Renewal
Step 1.a	For Main campus
	- Download Work Term Evaluation (WTE) form and submit to the CMC

	For Down Town Campus
	- Download WTE form - Employer and submit to the CMC.
	- Download WTE form - Students and submit to the CMC.

*Step 1.b	Download Academic Standing Exemption form (Main Campus students only) and submit to the CMC if you are a graduate student and your GPA is below 3.0.

Step 2.	Complete your online co-op renewal form

Step 3.	After the CMC receives your WTE, Academic Standing Exemption form (if applicable) and online co-op renewal application, it will process your co-op renewal and send an approval by email. Allow at least 8 business days for your renewal to be processed.

Step 4.	After you receive CMC approval by email, register for co-op.

International students only: Note: The CMC will forward your co-op renewal to the International Center and the International Center will process your co-op renewal ONLY after you register for the co-op course.
Step 5.	After you receive International Center approval by email pick up your updated I-20 Work Authorization.
