Please complete Steps 1-3 below to register for Internship. All paperwork must be signed by the student, the faculty
advisor and the work-site supervisor before it will be accepted by the Career Management Center. NO copies or faxes will be
accepted. A job offer letter that includes your start date, your title and your salary, and a detailed job description must
also be submitted with this paperwork. International students, please make sure that you complete the 2 page Faculty Advisor
Form with the added CPT Recommendation Form in Step 1. All other students must complete the Faculty Review of Job Description
Form in Step 1. At the end of each semester of Internship, you are required to submit a Work Term Evaluation in Step 4 to be
completed by you and your work-site supervisor.
